Thank you for choosing Mana Ola Spa. Our goal is to provide a peaceful, restorative wellness experience in a safe, professional, and welcoming environment. Please read the following information carefully before receiving your treatment.

For your safety, please review and complete the information below.

CONSENT TO TREATMENT
I voluntarily consent to receive spa services at Mana Ola Spa which may include, but are not limited to:

• Massage therapy

• Facials and skincare treatments
• Body treatments and wraps
• Aromatherapy
• Hydrotherapy services
• Other wellness services offered by the spa

I understand that these services are intended for relaxation and wellness purposes only and are not intended to diagnose, treat, or cure any medical condition. Spa services are not a substitute for medical care provided by a licensed physician or healthcare provider.

HEALTH DISCLOSURE
I acknowledge that I have completed the Spa Guest Wellness Intake Form accurately and to the best of my knowledge.
I understand that it is my responsibility to inform my therapist of any medical conditions that may affect my treatment, including but not limited to:

• Injuries or recent surgeries
• Allergies or sensitivities
• Pregnancy
• Skin conditions
• Changes in health or medications

I agree to notify the therapist immediately if any discomfort or pain occurs during the treatment so that adjustments can be made for my safety and comfort.

CONTRAINDICATIONS
Certain medical conditions may make spa treatments unsuitable or require modification. These conditions may include, but are not limited to:

• Fever, contagious illness, or infection
• Open wounds, burns, or skin irritation
• Recent surgery or injury
• Severe circulatory conditions or blood clots
• Certain heart conditions
• Pregnancy (without therapist awareness)

If any of these conditions apply, I agree to inform my therapist prior to treatment. Mana Ola Spa reserves the right to modify or decline treatment if it is determined that a service may be unsafe based on a guest’s health condition.

PROFESSIONAL BOUNDARIES
I understand that all spa services are professional wellness treatments. Any inappropriate behavior, language, or conduct toward spa staff will result in the immediate termination of the service and may result in refusal of future services.

RIGHT TO REFUSE SERVICE
Mana Ola Spa reserves the right to refuse or discontinue treatment if it is determined that a service may be unsafe or inappropriate based on a guest’s health condition or behavior.

LIABILITY RELEASE
I understand that participation in spa services carries certain inherent risks. I voluntarily assume responsibility for any personal health conditions and release Mana Ola Spa, its owners, employees, therapists, and affiliates from any liability for injury, allergic reaction, or adverse effect that may occur during or after spa services, except in cases of gross negligence.
I understand that results from spa treatments may vary and that no guarantees have been made regarding outcomes.
